Appointment Process: Return COMPLETED (double-check to make certain you have signed everywhere you are supposed to) appointment papers via mail including check for dues. UCT requires "wet" signatures.

  2. Membership Application/w $30 or $35(ADD coverage) fee
  3. State appointment fee (ID=$0.00, WA $20)
  4. General Agent Sales Agreement
  5. Business Addendum
  6. Commission Schedule
  7. Agent License
  8. Background Release (if applicable, i.e. UT)

Certification & Training: No certification or training. You need to be appointed and maintain your Insurance License Continuing Education requirements.

  • Effective immediately, to ensure the security of submitted applications, UCT has switched to a Web-based - rather than an e-mail based -submission system. Go to "For Agents" area on UCT's Web site: www.uct.org, click on the Online Application Submission button and follow the instructions to submit.
  • Continuing education (CE) credits for Medicare Part D for 2008 will only be available for Webinar or live training certification, not for online certification. Please visit the For Agents area of UCT's Web site for more details. Please note that CE credits will only be given to those agents working through a carrier, such as UCT. If you are appointed through your FMO, no CE credits will be given.
  • Effective Dec. 1, 2007, UCT will only accept the MS APP 0406 Medicare supplement applications from Arizona, Idaho and West Virginia. UCT will only accept the 2007 Medicare supplement applications from Louisiana, Nevada, Ohio and Virginia. We will also only accept the 0406 version of any world applications.
  • When submitting Medicare supplement applications, it's no longer necessary to include form MS-81 (UCT Application for Membership) or form FM-05 (UCT Application for Fraternal Membership). The information previously provided by these forms has been incorporated into the newer versions of the Medicare supplement applications. The forms must still be included with all life insurance applications, however.
  • Want to speed up the processing time on your Medicare supplement applications? Then complete the Client Medication Form and send it in with your application. The form isn't required, but it can help make the underwriting process quicker and get your policies issued faster. The form may be downloaded from the "For Agents" area of UCT's Web site or obtained by contacting the Customer Service Department at ext. 551 or at customerservice@uct.org.

Broker Support: Before you call anyone: Log-on (www.uct.org) they have a great website, filled with information, forms, applications, rates, commission information, etc. etc. First time users of the uct.org website: go to the "For Agents" tab. The login screen will be displayed, use your Agent number as the ID, this number is also your temporary password. The first time they log in you will be prompted to change your password and update a security question.
